The Teamsters Disaster Relief Fund is offering assistance to members living in Mississippi after severe storms, tornadoes, straight-line winds and flooding caused major damage to homes and property from January 20-21, 2017.

Available below are Teamster “Disaster Relief Alert Notices” and “Request for Help” forms. If you live in the affected area and experienced a loss due to this FEMA-declared major disaster, please download and submit the forms. Members living in the following Mississippi counties are eligible to apply: Forrest, Lamar, Lauderdale and Perry.

Be sure to include all requested documentation. The deadline for application is March 31, 2017.
